Hospice care focuses on improving the quality of life for persons and their families faced with a life-limiting illness. The primary goals of hospice care are to provide comfort, relieve physical, emotional, and spiritual suffering, and promote the dignity of terminally ill persons. Hospice care neither prolongs nor hastens the dying process.
Your Healthcare Team consists of the following people and services:
Nurses - pain control and symptom management, medical equipment
Social Workers - monetary assistance for medicines (based on family's income), counseling, end-of-life decision making
Chaplain - spiritual and emotional support
Volunteers - patient care, companionship, administrative support, bereavement, community education, Watchman Program
Home Health Aides - assist with activities of daily living
Homemaker - provides housekeeping support
Bereavement - grief support groups, open to any individual dealing with loss as a result of death
On Call - nurse on call 24 hours a day
