  • RN (Case Manager)

    JOB GOAL: 

    To provide professional nursing care to Hospice patients by utilizing all elements of the nursing process; and to initiate and apply appropriate preventative and therapeutic nursing procedures and techniques; and to maintain communication regarding patient care with the physician and supervisory personnel.


    MINIMUM QUALIFICATIONS:


    1. Must be licensed as a Registered Nurse through the State Board of Nursing; and


    2. Must be a graduate of an accredited school of professional registered nursing, BSN preferred; and


    3. Have had two (2) years med-surg experience with a hospice, home health, medical, surgical or critical care experience; and


    4. Possess and maintain a current CPR Certification; and


    5. Must have and maintain current driver’s license and current automobile liability insurance.


    6. Must successfully complete the post job offer physical and submit to periodic tuberculin skin tests or chest x-rays at agency expense.


    ADDITIONAL DESIRABLE QUALIFICATIONS:


    1. Experience with providing good pain management, IV skills preferred; and


    2. Committed to the Hospice philosophy and principles of death/dying; and


    3. Must be flexible and cooperative in fulfilling role obligation (24/7), and have the ability to work within an Interdisciplinary Team; and


    4. Have had home health care or nursing home experience desired; and


    5. Possesses and maintains good physical stamina and mental health; and


    6. Complies with accepted professional standards and principles, and has the ability to work within an Interdisciplinary Team.


    ESSENTIAL FUNCTIONS

    1. Seeks and obtains sufficient information necessary to function effectively in the Hospice care settings where patient/family/caregivers are requiring care.


    2. Provides professional nursing care by utilizing all elements of nursing process.


    3. Assesses and evaluates patient’s status by:


    a. Writing and initiating Plan of Care


    b. Regularly re-evaluating patient/family/caregiver needs


    c. Revising the Plan of Care as necessary


    4. Initiates and sustains as needed the implementation of orders for medications and treatments as prescribed by the physician in the physician’s Plan of Care.


    5. Initiates and applies appropriate preventative and therapeutic nursing procedures and techniques and maintains communication regarding patient care with the physician and supervisory personnel.


    6. Completes, maintains and submits accurate and relevant clinical notes regarding patient’s condition and care given.


    7. Uses supplies and equipment effectively and efficiently.


    8. Observes the patient for changes in condition and reports these changes and the patient’s/family’s/caregiver’s needs to the attending physician and appropriate supervisory personnel. Records pain/symptom management changes/outcomes as appropriate.


    9. Assists the patient with the activities of daily living and facilitates the patient’s efforts toward self-sufficiency and optimal comfort care.


    10. Teaches the patient/family/caregiver self-care techniques as appropriate. Educates patient/family/caregiver regarding “‘end of life” issues as appropriate. Provides medication, diet and other instructions as ordered by the physician and recognizes and utilizes opportunities for health, counseling with patient/family/caregiver. Works in concert with other Hospice and Interdisciplinary Team members.


    11. Provides and maintains a safe environment for the patient.


    12. Assists the patient/family/caregiver and other team members in providing continuity of care.


    13. Works in cooperation with the family/caregiver and Hospice Interdisciplinary Team members to meet the emotional needs of the patient/family/caregiver.


    14. Supervises ancillary personnel and delegates responsibilities when required.


    15. Performs within the limits of preparation and experience.


    16. Accepts responsibility of assignment to perform a specialized procedure, such as IV Therapy, only when qualified with specialty training, proven competency, clinical background and expertise in the specialty area.


    17. Assumes responsibility for personal growth and development and maintains and upgrades professional knowledge and practice skills through attendance and participation in continuing education and in-service classes.


    18. Fulfills the obligation of requested and/or accepted case assignments.


    19. Actively participates in Quality Management program.


    20. Provides back-up on-call availability as assigned by the Clinical Care Manager.


    21. Shares coverage of on-call services 24 hours a day, if necessary.


    22. Satisfactory return demonstration of the following skills: central line catheter, implanted ports, gastrostomy tube maintenance.

  • Accounting Clerk

    Job Goal: To manage all aspects of accounts payable and be the back-up to the Billing Specialist. This role is critical in the revenue cycle process of the organization.

    Minimum Qualifications Required:


    Education: High School Diploma. Associates degree in related field preferred.


    Experience: Minimum of 2 years of accounts payable experience required. Minimum of 1 year of medical billing with billing software or EMR experience preferred. Previous experience with QuickBooks preferred.

    · Strong interpersonal skills to build relationships with co-workers, leadership, and HOTV patients and families.


    · Must be able to connect with people through understanding and compassion.


    · Ability to react calmly and effectively in all situations.


    · Ability to maintain confidentiality of sensitive information.


    · Understanding of hospice philosophy, including understanding of death, dying, and the grief process.


    · Must be able to communicate clearly, professionally, and with empathy.


    · Self-motivated and able to work independently to perform detailed work in a timely manner with accuracy.


    · Proficient with computer software such as word processing, spreadsheets, and data entry


    · Must maintain current driver’s license and automobile insurance.


    · Must successfully complete pre-employment physical and drug screen and submit to immunizations and periodic tuberculin skin tests or chest x-rays and company expense.


    Essential Functions:


    1. Responsible for receiving, verifying, coding, and entering all invoices into Quick Books on a weekly basis.


    2. Prepare weekly payment batches including printing checks, ACH payments, and/or wire transfers.


    3. Prepares daily bank deposits.


    4. Reconcile vendor statements to accounts payable sub-ledgers and resolve discrepancies.


    5. Responsible for vendor files and annual process for transfer to long-term storage.


    6. Responsible for petty cash and monthly reconciliation.


    7. Assists in collection of requested documents for annual company audit.


    8. Receives insurance verification request from intake and confirms patient insurance/payer coverage and benefit period requirements.


    9. Rotates with department staff to cover insurance verifications on weekends.


    10. Keeps daily record of admissions and deaths and submits required documents for both.


    11. Reconciles admissions and deaths on a monthly basis.


    12. Files Notice of Election within 5 days of admission.


    13. Knowledgeable in the patient billing and claims submission process to act as back-up to the Billing Specialist.


    14. Assists with incoming revenue and paperwork for fundraisers and memorials.


    15. Back-up to entering client information in Bloomerang.


    16. Back-up to post office runs and distribution of mail.


    17. Rotates at least one day per week to cover the front desk during receptionist breaks and/or vacations.


    18. Adheres to work schedule and maintains good punctuality and attendance.


    19. Responds to internal and external email, phone, qliq, and verbal inquiries in a timely manner.


    20. Understands that teamwork is a crucial part of hospice care, and can work well with colleagues, no matter the circumstances. Using individual skills, cooperating with others, and accepts and provides constructive feedback in a professional manner.


    21. Other duties as assigned.


    Continuing Education Requirements:


    Hospice personnel are expected to participate in appropriate continuing education as may be requested and/or required by their immediate supervisor. In addition, Hospice personnel are expected to accept personal responsibility for other educational activities to enhance job related skills and abilities. All personnel must attend or complete assigned mandatory educational programs and/or annual on-line courses.


    The days worked for this role will be Monday, Tuesday, Thursday, and Friday.

  • Patient Care Services Director

    Job Goal: To provide supervision and management of patient care staff by promoting effective performance and quality care through supporting and guiding staff. This role is a critical part of the organization’s leadership team and will be responsible for operations and key metrics of the patient care staff and represent the department in meetings.


    Demonstrates Hospice of the Valley’s mission, vision, and values through behavior and actions.


    Minimum Qualifications Required:


    Education: Current license as a registered nurse through the Alabama State Board of Nursing. Bachelor's degree, or Associate’s degree with 4 years of management experience, required. Certification in Hospice and Palliative Nursing (CHPN) highly desired.


    Experience: At least 4 years of hospice or home health experience and at least 2 years of staff management experience required.

    · Must successfully complete pre-employment physical and drug screen and submit to immunizations and periodic tuberculin skin tests or chest x-rays and company expense.


    Essential Functions:


    1. Manages day to day operations of the patient care department and staff including Clinical Supervisor(s) RN Case Managers, CNAs, On-Call RNs, Admission RNs, Social Workers, and Clinical Chaplain.


    2. Supervises the delivery of hospice services provided to the patient and family.


    3. Supports staff through encouragement, guidance, and hands-on direction when needed.


    4. Responsible for conducting supervisory visits with all clinical staff on an annual basis.


    5. Participates in admin on-call rotation with other clinical leaders to provide 24/7 admin support.


    6. Available to perform patient visits if needed or assist staff during difficult patient visits.


    7. Attends and monitors weekly IDT meetings to ensure care is appropriate to patient diagnosis and care goals, care is appropriate according to hospice scope of practice, and care is provided in accordance with established compliance standards.


    8. Monitors key metrics on an on-going basis, reporting monthly to the CEO.


    9. Reviews all clinical vendor contracts annually and updates as necessary.


    10. Reviews all assigned policies and procedures quarterly and submits edits by designated deadline.


    11. Participates in quarterly QAPI meetings with review of assigned metrics.


    12. Promotes community awareness through participation in public relations activities and/or educational or community events.


    13. Monitors department budget, overtime, and resource utilization.


    14. Reconciles monthly medication and DME invoices for accuracy prior to submission for payment.


    15. Receives patient complaints and follows complaint and grievance reporting process.


    16. Participates in recruitment, interviewing, hiring, and orientation of new clinical staff.


    17. Participates in quality improvement assessments and initiatives.


    18. Monitors compliance with CMS, State licensure regulations, and State and Federal standards.

  • Hospice Educator

    Hospice of the Valley has a career opportunity for a full-time (72 hours) Hospice Educator position.  This position will develop business partnerships generating hospice referral growth by making effective sales contacts, calls, and presentations.  This position will also serve as the HOTV staff liaison with physicians, hospital/institutional nurses, social workers, discharge planners and community agencies.  The Hospice Educator position is responsible for the overall development and referral growth of assigned territory.   This position also participates in marketing efforts for the purpose of educating the healthcare community about hospice programs, including physician calls, lunch and learns, health fairs, speaker’s bureau, and CEU programs for healthcare providers.


    MINIMUM QUALIFICATIONS:

     

    1. Experience in marketing or customer service; an associate's degree in nursing: or related field; or equivalent professional experience; and

    2.  Minimum of two (1) years of experience in hospice or home health sales or a related industry with a proven record of achieving incremental growth in direct sales; and

    3.  Must possess and maintain a current Alabama driver’s license; and

    4.  Must possess and maintain liability insurance on vehicle; and

    5.  Must successfully complete the post job offer physical and submit to periodic tuberculin skin tests or chest x-rays at agency expense.


    ADDITIONAL DESIRABLE QUALIFICATIONS:

     

    1. Must be proficient in the use of Microsoft programs for internal and external communications, presentations, database tracking/reporting, and spreadsheet applications.

    2. Must have excellent oral communication skills. Public speaking will be required for this position.

    3. Is self-directed and possesses the ability to work with little supervision
